Park Avenue Medical was founded in 2012 by Dr. Anace Said and Nada Shaarbaf.

Their goal was to create a medical practice where Wallingford patients could experience world-class care close to home, where it matters. The office grew over the years, becoming the full team of expert clinicians it is today. With years of experience in the medical field, our professional primary care providers have taken pride in providing the highest quality services for patients in Wallingford, North Haven, Middletown, Cheshire, Meriden, CT, and the surrounding areas.

I graduated from Baghdad School of Medicine, and I’ve been practicing for 26 years as an internist. I moved to NYC in 1997, where I completed my training before moving to Connecticut. We founded Park Avenue Medical in 2012 to bring quality care to the people of Wallingford, and I’ve loved every minute since! In my free time, I like watching movies with my family and biking. I look forward to treating you!

I have been a nurse for 25 years, and I earned my Master's Degree as a family nurse practitioner in 2015. I began at Park Avenue Medical as a nurse practitioner student and never left! When I'm not at work, I enjoy spending time with my family, traveling, and curling up with a good book!

I spent many years working at several different jobs. My only wish is that I found this job earlier in my life. Dr. Said is the type of doctor that is very caring and devoted to his patients as well as his staff. He has picked out a variety of devoted and educated APRN to make sure his quality of care is maintained. I have never had a job that I look forward to going to every day. We are a team but more than that they are my work family.

HILARY SULLIVAN

I graduated from UConn with a degree in health, culture, and society and from QU with a bachelor's degree in nursing. In 2019, I graduated from Georgetown University’s family nurse practitioner program. I did my first clinical rotation here, and the level of caring and clinical excellence struck me immediately. The staff has become my second family, and I genuinely look forward to coming to work. I enjoy reading, traveling, attending concerts/shows, and spending time with my family and pets!

CHELSEA SONDAK

I went to the University of Hartford where I obtained my bachelor's of health science, and I later obtained my bachelor's in nursing from MCPHS University. I have been an RN since 2015 with experience in many different areas within acute care. I also graduated with my master’s degree as a family nurse practitioner from Herzing University in 2021. When I'm not at work I enjoy going for a hike with my family and being with my two rescue dogs!

I graduated from Mount Saint Mary College in 2014 with a Bachelor of Science in Nursing (BSN). I graduated with my Master of Science in Nursing: Family Nurse Practitioner degree in 2021. I was lucky enough to become a nurse practitioner student at Park Avenue Medical. I have learned so much working with Dr. Said and my remarkable co-workers. In my free time I like to hike with my husband, go to dinner with family & friends, and spend time with my Goldendoodle & Maltipoo! I also love to run.
